Abstract / Description of output

The originally published version of this article contained the following error. In the footnotes for tables 1-3, the following line appears 'SESincludes individual socioeconomic indicators, familial support, smoking and driving.' This was incorrectly rendered during the proof process and has since been corrected to read "SES plus includes individual socioeconomic indicators, familial support, smoking and drinking." The publisher apologizes for this error.
